Unity Healthcare is a subspecialty diabetes, endocrinology and internal medicine clinic in Surrey, BC, providing timely, evidence-based consultation for patients with complex metabolic and chronic conditions. We work closely with family physicians, nurse practitioners and other specialists to deliver coordinated care plans that are practical in real-world primary care settings.

How to Refer
Unity Healthcare currently accepts referrals from family physicians, nurse practitioners and other specialists in British Columbia as well as from other provinces/ territories in Canada.
- Please fax referrals to: +1-604-545-1299
- Include: patient demographics, relevant history, medications, recent labs and imaging, and any specific clinical questions.
- For urgent issues (e.g., suspected adrenal crisis, severe hypercalcemia, rapidly progressive thyroid eye disease), please mark the referral as “Urgent” and clearly state the concern.

Triage & Wait Times
Referrals are triaged based on urgency and complexity. While exact wait times vary, we prioritize:
- New-onset Type 1 diabetes and unstable Type 2 diabetes
- Pregnancy-related diabetes or endocrine issues
- Severe symptomatic hyper- or hypothyroidism (including Graves’ disease)
- Suspicious thyroid nodules or endocrine cancers
- Adrenal and pituitary emergencies or high-risk lesions
Where appropriate, patients may be seen in collaboration with our pharmacist and allied health partners to accelerate initiation of continuous glucose monitoring, injectable therapies or bone treatments.
What We Help With
Unity Healthcare is a specialist clinic and we see patients by referral. To become a patient here:
- Complex Type 2 diabetes (including multi-morbid, polypharmacy, frequent hypoglycemia)
- Type 1 diabetes and LADA
- Diabetes in pregnancy (gestational, pre-existing T1/T2)
- Medication-induced hyperglycemia (e.g., oncology, steroids, transplant)
- Guideline-based pharmacotherapy for obesity
- Weight management in patients with diabetes, cardiovascular disease or MASLD
- Evaluation of weight gain related to endocrine or medication causes.
- Osteoporosis and high fracture risk
- Complex anti-resorptive / anabolic therapy decisions
- Secondary fracture prevention and injection pathways with pharmacy support.
- Hyperthyroidism, including Graves’ disease
- Hypothyroidism with refractory symptoms
- Thyroid nodules and thyroid cancer, including ultrasound-guided biopsy (ECNU-certified)
- Primary hyperparathyroidism, hypocalcemia and other calcium disorders
- Adrenal incidentalomas, Cushing’s, primary aldosteronism
- Pituitary lesions and pituitary hormone abnormalities
- Hypogonadism, PCOS and endocrine aspects of reproductive disorders
- Complex dyslipidemia and high-risk cardiometabolic profiles.

Research & Clinical Trials (for Providers)
Unity Healthcare is a site for multiple phase 2 and 3 clinical trials in:
- Type 2 diabetes (including GLP-1/GIP-based and other novel therapies)
- Type 1 diabetes (beta-cell preservation and adjunctive therapies)
- Obesity in patients with and without diabetes
- Graves’ disease and other autoimmune thyroid conditions
- Dyslipidemia and cardiovascular risk reduction.

Referring providers continue to participate in ongoing care. Trial participation does not replace routine primary care or specialist follow-up; instead, patients receive additional monitoring within protocol. We share relevant results and treatment changes with you throughout the study.
